ACCOUNTABILITY FORM

The accountability form is essential for your coach as it provides critical information about your current health, habits, and progress. Your coach will use this to make adjustment to your plan so that your plan always matches your current lifestyle needs and goals.

It’s really important that you complete the form at least one day before your appointment. Your coach will prepare for your call the day before, and having your form will help them lead you to succeed.

We know everyone is very busy, and it’s easy to forget. So, to help you remember, we’ll be emailing you, and sending a text reminder, 24 hours before your scheduled call. In that message, you’ll also find a link to the accountability form.

If we don’t receive your completed form one day prior, we may need to reschedule our call. So, don’t forget to fill it out! 😉

When you have your weekly check-in call, it’s best to be in a distraction-free environment where you can take notes and have the option to interact within the CSF app.

Afterward, your coach will email you with lots of helpful information from the call. This includes details on how well you’ve been sticking to your plan, thoughts on how you’re doing, and suggestions for improving your adherence. Plus, your coach will set you up for success with an action plan that helps keep you focused for the week ahead.
